This is an improvement for searches on initial substrings.  The substring index has no information about whether a substring appears at the beginning of the attribute value, so evaluation of an initial substring now prefers a cursor through the equality index rather than using the substring index.  This yields a smaller number of entries to be fetched and filtered.
